Enhanced Care Management (ECM)
At NJs Forward Arrow, our Enhanced Care Management (ECM) services are designed to provide whole-person, coordinated care for individuals with complex health and social needs. We take a person-centered approach to ensure that every client receives the support, advocacy, and guidance they need to improve their health and quality of life. Our team is bilingual and understands the importance of community in the language our clients are most comfortable with.

What is Enhanced Care Management?
Enhanced Care Management is a Medi-Cal benefit that provides intensive, community-based care coordination for people who face difficult health and social challenges. ECM focuses on individuals who often struggle to access and manage care due to factors like chronic illness, mental health conditions, substance use disorders, or housing instability.
Who We Serve
We provide ECM services to eligible individuals, including:
- People experiencing homelessness or housing insecurity
- Adults and youth with serious mental illness (SMI) or substance use disorder (SUD)
- Individuals with complex chronic health conditions
- Birth Equity Services for pregnant women and post-partum support
- Children and youth with complex physical, behavioral, or developmental needs
- High utilizers of emergency and hospital services

- Care Coordination – We connect you with medical providers, mental health professionals, housing services, and community resources.
- Health System Navigation – We help you access and understand the health care system, including appointments, benefits, and medications.
- Support During Transitions of Care – Whether you’re leaving the hospital, a treatment facility, or incarceration, we ensure a smooth and supported transition.
- Advocacy & Ongoing Support – Our care managers stay connected, advocating for your needs and making adjustments to your care plan as needed.
What are Community Supports
- Non-Medical Services That Improve Health Outcomes
- Community Supports are available at no extra cost to Medi-Cal members who qualify. These services are designed to address the non-medical factors that affect health—such as housing, nutrition, and personal care needs.

- Housing Transition & Navigation – Helping you find and apply for safe, stable housing
- Housing Tenancy Support – Assistance with maintaining housing, landlord issues, and budgeting

- Housing Deposits – One-time support for security deposits, application fees, and essential furnishings
- Short-Term Post-Hospitalization Housing – Temporary housing to help you recover safely after discharge
- Medically Tailored Meals & Nutrition Support – Healthy food delivered based on your health needs
- Environmental Accessibility Adaptations – Home modifications to support independent living
- Personal Care & Homemaker Services – Help with daily living tasks such as cooking, cleaning, or dressing
- Respite & Day Habilitation
- Sobering Center Referrals – Safe spaces for recovery and stabilization
Eligibility
You may be eligible for Community Supports if:
- You’re enrolled in a participating Medi-Cal managed care plan
- You have complex medical or behavioral health conditions
- You face barriers such as homelessness, food insecurity, or limited mobility
