What is Enhanced Care Management?
If you are covered by Medi-Cal, you have a benefit that is free and included in your health plan that can be helpful in providing you with additional support.
Enhanced Care Management (ECM) is for members with complex medical and social needs. ECM helps members pair with a lead care manager who helps walk you through a care plan to determine what you need the most, sometimes that could be extra help applying for housing vouchers, finding a new place to live, making doctors’ appointments or help with your medical appointments.
If you qualify for ECM, you will have your own care team with a Lead Care Manager. This person will work with you and your doctors, specialists, and others. We will make sure that everyone works together to develop the best care plan for you!

Navigating the healthcare system can be challenging, especially for individuals with complex needs. These difficulties can create additional stress and uncertainty. At NJS Forward Arrow we understand the importance of providing compassionate guidance and strong advocacy to support members throughout their healthcare journey.
Our lead care managers are your partners and work as a team alongside you, helping you understand health conditions, treatment options, and seek out referrals for resources, such as housing options and nutritious meal delivery. Our goal is to ensure you feel heard and understand your rights within the healthcare system.
Birth Equity Services
At NJS Forward Arrow, we believe every birthing individual deserves safe, respectful, and empowering care—no matter their background, identity, or circumstances. Our Birth Equity Services are designed to confront the disparities that exist in maternal and infant health and ensure that every family has access to the support they need for a healthy start. We help you with setting up appointments, applying for benefits, transportation and being there for you as a support when you need it the most.
Birth equity is more than a concept—it’s a commitment. We work to eliminate biases, remove barriers, and champion practices that honor the dignity, culture, and choices of each person we serve. Through advocacy, education, and personalized care, we help families navigate pregnancy, childbirth, and postpartum with confidence and protection.
